Aged and community care services
Home and Community Care (HACC)
This program provides basic maintenance and support services. It helps frail aged people who have an ongoing functional disability which impacts on their ability to manage tasks of daily living. The program also supports younger people with disabilities and their unpaid carers to continue living independently in the community.
Aged care assessment
Assessments to determine the level of care and services needed by frail older Western Australians are performed by health professionals within an Aged Care Assessment Team (ACAT). These teams provide access to services tailored to your needs when you are no longer able to manage living at home without assistance.
Transition care services
- Transition care program provides short-term care in a residential or community setting that older people can receive to help their rehabilitation after a hospital stay.
- Care awaiting placement helps older people who are ready to leave hospital but are awaiting a residential placement.
Continence management advice
This service provides community-based management and advice for people with ongoing conditions who are financially disadvantaged and cannot afford continence products.
